We are pleased to announce that the Residents Section of CTSNet has undergone a major renovation that should allow for easier use and access to important websites. We are also pleased to announce that several new sections have been created which are directed towards cardiothoracic and general surgery residents as well as medical students. These include links for resident education, program profiles, AHA/ACC guidelines, and updated links to important associated cardiothoracic surgery websites. Future goals include an e-video library of basic cardiothoracic surgical techniques and an updated meeting event link. We are interested in feedback to continue to improve and make this the major integrative site for those interested in cardiothoracic surgery.

Off-Pump vs. On-Pump CABG Study
McMaster University and the Population Health Research Institute (PHRI) at Hamilton General Hospital is currently conducting a 5000 patient, 5-year prospective, randomized, multinational, clinical trial that is comparing On-Pump CABG to Off-Pump CABG techniques (CORONARY). Support for this study is from the Canadian Institute of Health Research. We would like to recruit a few more centers with experience and expertise in On-Pump and Off-Pump CABG, either as a group or as individual surgeons. Register Now to Attend the 2009 Legislative Advocacy Workshop
The STS Legislative Advocacy Workshop is back—just as Congress is in the midst of debating health care reform legislation. Designed specifically to develop leaders who can take a stand on health care policy, the timing for the Advocacy Workshop is perfect. Plan now to attend and help shape the health care debate on behalf of cardiothoracic surgery!
The 2009 Legislative Advocacy Workshop will be held September 13-15 in Washington, DC. The anticipated congressional calendar calls for the culmination of health care reform legislation to be the major order of business when elected leaders return from their August recess. The Workshop will provide an extraordinary opportunity for STS members to participate in the history-making debate in Washington over health care reform legislation.
The Workshop will kick off with a Sunday evening reception, followed by a day of discussions with congressional leaders and Obama administration policy makers. Following an in-depth update on critical issues, an advocacy training session will help participants convey the specialty’s message. Appointments will then be scheduled for Tuesday morning as attendees hit Capitol Hill to meet with congressmen and senators to discuss the challenges facing cardiothoracic surgeons. Advances in Quality & Outcomes 2009 – Register Now
Surgeons and data managers interested in improving the quality of both data collection and patient care should attend the upcoming Advances in Quality & Outcomes: A Data Managers Meeting to be held September 24-26 at the Marriott Hotel and Marina in San Diego, California.
The AQO Conference, now in its sixth year, has become the definitive meeting for STS Database professionals. Participation at AQO, like participation in the STS National Database, demonstrates a commitment to quality improvement and is an investment in the future.
In 2009, AQO attendees will benefit from in-depth presentations by data managers and surgeons who will provide practical applications for data collection and achieving quality outcomes. With the national spotlight increasingly on the potential use of databases, AQO offers an important opportunity for data managers to bring home new perspectives that can benefit their employers.

FDA Panel Recommends Approval of the Medtronic Melody® Transcatheter Pulmonary Valve under HDE
This month, the U.S. Food and Drug Administration (FDA) Circulatory System Devices Panel unanimously recommended conditional approval of a Humanitarian Device Exemption (HDE) for the Melody Transcatheter Pulmonary Valve. The panel’s recommended conditions of approval include a post-approval study, revision of the patient brochure and instructions for use, and implementation of a physician training and education program. The Melody Transcatheter Pulmonary Valve provides a less invasive means to address the effects of a dysfunctional pulmonary valve for children and adults with congenital heart disease.
Melody is on track to become the first transcatheter heart valve of any kind to receive FDA approval for use in the US; however, following the panel recommendation, the Melody Transcatheter Pulmonary Valve remains an investigational device in the US, where its use is limited to a clinical trial approved by the FDA. Approval of the Melody system as an HDE is under review by the FDA.

The National Lung Cancer Partnership is pleased to announce our 5th Annual Research Grant competition. This year, four, 2-year $100,000 awards are available to clinical and basic science fellows and junior faculty to advance their research in lung cancer etiology, prevention, early detection, treatment, and symptom management. Three grants will be co-funded with the LUNGevity Foundation, with the assistance of Genentech and Sanofi-aventis, and one grant will be funded by the North Carolina Lung Cancer Partnership.
The purpose of these awards is to drive forward research that will increase understanding of lung cancer risk, biology, and response to treatment, in an effort to fulfill the Partnership’s mission of decreasing deaths due to the disease and helping patients live longer and better lives.
At the time of application, an applicant must hold a doctoral degree (MD, PhD, DO, DrPH, or equivalent), and be a post-doctoral fellow or within the first 5 years of a faculty appointment at a not-for-profit institution in the United States or Canada. Applications addressing sex differences in lung cancer are particularly encouraged. Applicants will be judged on the merits of their research proposal, career development plan, and research environment.

Long Term Results of Leaflet Extension Technique in Aortic Regurgitation: Thirteen Years of Experience in a Single Center.

In the July Annals, Dr. Dong Seop Jeong and colleagues from Seoul National University, South Korea, examine their results with aortic valve repair using leaflet extension with pericardium. The technique is nicely illustrated in the paper. Between 1995 and 2004, 41 patients with aortic insufficiency were included (mean age 32.2 years). The AI was rheumatic in 75.5%, degenerative in 4.9%, bicuspid aortic valve in 9.8%, infective endocarditis in 2.4%, and congenital in 7.3%. 28 leaflet extensions were performed in three leaflets for 29 patients, two leaflets for 3 patients, and only one leaflet for 6 patients. The mean follow-up duration was 92.9 +/- 48.4 months. There were no early deaths and there were 2 late deaths (one valve related). During a mean follow-up of 7 years, important AI was seen in 7 of 41 patients. The causes of recurrent AI were endocarditis (3), disease progression in (3), and Behçet’s disease (1). There were 6 reoperations, 3 due to endocarditis, 2 for disease progression, and 1 for suture dehiscence associated with Behçet’s disease. Cumulative survival was 92.6% at 13 years. Freedom from recurrent AI was 97.5% at 5 years, 81.7% at 10 years, and 68.1% at 13 years. The authors concluded that long-term durability of the leaflet extension technique was acceptable. The reoperations increased with time, but pericardial leaflet dysfunction was not the cause.
The results reported herein seem competitive with valve replacement when patient characteristics are taken into account. Yet, this technique (which has been available for several decades) has not met with overwhelming acceptance world wide. The 2nd International Minimally Invasive Thoracic Surgery Summit will be held October 9-10, 2009 in Boston, Massachusetts USA
This summit is the first to bring together a group of thoracic surgeons dedicated to the field of minimally invasive general thoracic surgery. The goal is to present the current state-of-the-art and give insights and data based on the faculty's experience on where the field is heading.
The agenda discusses a range of topics presented by leaders in the field. Pertaining to lung cancer, these include minimally invasive approaches to lung cancer treatment, lung cancer screening, and new developments in lung cancer molecular biology. The workup and staging of esophageal cancer and benign esophageal disorders as well as the best treatment for esophageal malignancy are highlighted.
Minimally invasive approaches to the mediastinum are reviewed. For each area, the potential for application of the robot is featured. A fascinating section focuses on new developments in technology, including stent therapy, navigational bronchoscopy, lung localization options, sentinel node identification in lung cancer, and ablative and innovative radiation techniques.
